Get Your 329 Card

Follow these simple steps to apply for your 329 card for medical cannabis (formerly known as a "blue card").

Step 1

Before completing attached forms or scheuding an appointment please call or text to verify you are eligible for a 329 card evaluation.

Step 2

Next you will need to fill out an online application on the https://medmj.ehawaii.gov/medmj/welcome

Under the field asking for the "MD/APRN-RX name", you need to start by entering my name as: 

First- Stacy

Last- Kracher

or Hawaii License # APRN-717

Step 3

Please upload to the DOH site a copy of your ID and any signed grow site or caretaker certificates.

At the end of filling out the online application for the Hawaii State Department of Health's (DOH) Marijuana Registry, you will be required to submit  $38.50 ($35 Application Fee for 1-year and $77.00 for a 2-year certification.  

Payment can be made with a credit or debit card, or direct withdrawal from a savings or checking account. 

Step 4

Print and submit Your Application.

I will meet with you and certify your your already established qualifying condition at the time of your appointment.

Once completed I will submit your application to the DOH for processing. Patients can not submit their own application directly to the DOH.
